| Referral Process
The referral process at Shands Rehab Hospital facilitates successful
transition into our program. These steps are in place for the safety of the patient and to ensure
that the patient is far enough along in recovery to begin a rehab program.
The referral process:
- The referring physician writes a patient evaluation for admission to
Shands Rehab Hospital.
- The physician contacts the nurse liaison at 352.265.8938 about the referral.
- The patient is evaluated within 24 hours of initial notification. The patient
must be medically stable prior to discharge from the referring hospital.
At the time of evaluation, the nurse liaison works with the referral source with regard to bed
availability, financial arrangements and appropriateness for admission. A proposed discharge date
from the referral source facilitates the transfer.
- Once the approval for transfer has been processed, discharge planning occurs
between the referring case manager or discharge planner and the nurse liaison.
A copy of the physician's orders and the discharge summary (one of which must include
medication orders) must accompany the patient and be given to the admitting nurse at Shands
Rehab Hospital.
For out-of-town referrals, the physician's orders will be faxed to the nurse liaison prior
to patient transfer.
