University of Florida Primary Care offers quality care for the adult patients in 2 convenient locations.

UF physicians offer patients eighteen years and older a wide range of services, including:

• family planning
• general and family physicals
• adult immunizations and tetanus shots
• preventative care and education
• cancer screenings
• minor surgery
• women’s healthcare
• dermatological consultations
• management of acute illnesses
• medication consultations

UF physicians who specialize in internal medicine staff the group practice. The University of Florida Primary Care Lake City- SW physicians are certified by the American Board of Internal Medicine.

Physicians in the West practice are board-certified family medicine doctors and care for both adults and children.

Appointments

Your initial visit at the clinic will be comprehensive. Please arrive 15 minutes before the scheduled appointment to complete the necessary paperwork.  The clinics offer free, on-site parking.

Please bring the following to your first appointment:

• any previous medical records
• immunization records
• current insurance card (including Medicare or Medicaid cards)
• list of allergies
• list of current medications and dosages
• list of other physicians and addresses
• list of questions for the healthcare team

Patients should bring all current medications to each office visit in order to help track medications and avoid duplicating or prescribing incompatible medications.

Cancellation

University of Florida Primary Care Lake City requires 24-hour notice of cancellation and ask that patients give the office notice as soon as possible to reschedule an appointment.

Office Hours and Scheduling

UF Primary Care Lake City is open Monday through Friday from 8:00 a.m. to 5:00 p.m. Patients are seen by appointment only. If the situation is urgent, please inform the staff when you call. Every effort will be made to see patients as quickly as possible.
